Exercise or physical activity are recommended options within stepped-care treatment models for depression. However, few physicians present these options to patients, in part because of the impression that the supporting evidence is weak or inconsistent. We speculate that the coocurrence of “counter-messaging” and deficient critical appraisal may lead to such impressions. We focus on TREAD-UK (ISRCTN16900744), the largest trial to investigate “whether physical activity can be an effective treatment for depression within primary care”. In media statements, researchers declared that exercise was ineffective in lowering depression. We examined (a) the results of the trial, critiques, and rejoinders, (b) the impact on internet searches, and (c) whether TREAD-UK was critically appraised, as reflected in citing articles. We show that the results of TREAD-UK were misrepresented. The media campaign resulted in a fourfold increase in relevant internet searches. Of articles characterising the results, 57% adopted the interpretation that exercise failed to lower depression, whereas only 17% were critiques. We identify similarities to media portrayals of the OPERA (ISRCTN43769277), DEMO (NCT00103415), and DEMO-II trials (NCT00695552). We note a disconcerting trend of media campaigns that misrepresent the effects of exercise on depression and call for increased scrutiny in peer reviewing both pre- and post-publication.  相似文献

Individuals differ in the intensity of exercise that they prefer and the intensity that they can tolerate. We examined the psychometric properties of the Preference for and Tolerance of the Intensity of Exercise Questionnaire (PRETIE-Q; Ekkekakis, Hall, & Petruzzello, 2005: Journal of Sport and Exercise Psychology, 27, 350 - 374). A sample of 601 college women (mean age 20 years) completed the PRETIE-Q and the Godin Leisure-Time Exercise Questionnaire. Both the Preference and the Tolerance scale were internally consistent (alpha coefficients of 0.89 and 0.86, respectively), with all items making positive contributions. A confirmatory factor analysis showed that model fit was reasonable. Both Preference and Tolerance were related to the frequency of participation in strenuous exercise and the total leisure-time exercise score. The PRETIE-Q appears to be an internally consistent and structurally valid measure, with a broad range of possible applications in exercise science.  相似文献

A model for systematic changes in patterns of inter-individual variation in affective responses to physical activity of varying intensities is presented, as a conceptual alternative to the search for a global dose?–?response curve. It is theorized that trends towards universality will emerge in response to activities that are either generally adaptive, such as moderate walking, or generally maladaptive, such as strenuous running that requires anaerobic metabolism and precludes the maintenance of a physiological steady state. At the former intensity the dominant response will be pleasure, whereas at the latter intensity the dominant response will be displeasure. In contrast, affective responses will be highly variable, involving pleasure or displeasure, when the intensity of physical activity approximates the transition from aerobic to anaerobic metabolism, since activity performed at this intensity entails a trade-off between benefits and risks. Preliminary evidence in support of this model is presented, based on a reanalysis of data from a series of studies.  相似文献

Abstract

The determination of the ventilatory threshold has been a persistent problem in research and clinical practice. Several computerized methods have been developed to overcome the subjectivity of visual methods but it remains unclear whether different computerized methods yield similar results. The purpose of this study was to compare nine regression-based computerized methods for the determination of the ventilatory threshold. Two samples of young and healthy volunteers (n = 30 each) participated in incremental treadmill protocols to volitional fatigue. The ventilatory data were averaged in 20-s segments and analysed with a computer program. Significant variance among methods was found in both samples (Sample 1: F = 11.50; Sample 2: F = 11.70, P < 0.001 for both). The estimates of the ventilatory threshold ranged from 2.47 litres · min?1 (71%[Vdot]O2max) to 3.13 litres · min?1 (90%[Vdot]O2max) in Sample 1 and from 2.37 litres · min?1 (67%[Vdot]O2max) to 3.03 litres · min?1 (83%[Vdot]O2max) in Sample 2. The substantial differences between methods challenge the practice of relying on any single computerized method. A standardized protocol, likely based on a combination of methods, might be necessary to increase the methodological consistency in both research and clinical practice.  相似文献

This article introduces a new theory, the Affective–Reflective Theory (ART) of physical inactivity and exercise. ART aims to explain and predict behavior in situations in which people either remain in a state of physical inactivity or initiate action (exercise). It is a dual-process model and assumes that exercise-related stimuli trigger automatic associations and a resulting automatic affective valuation of exercise (type-1 process). The automatic affective valuation forms the basis for the reflective evaluation (type-2 process), which can follow if self-control resources are available. The automatic affective valuation is connected with an action impulse, whereas the reflective evaluation can result in action plans. The two processes, in constant interaction, direct the individual towards or away from changing behavior. The ART of physical inactivity and exercise predicts that, when there is an affective–reflective discrepancy and self-control resources are low, behavior is more likely to be governed by the affective type-1 process. This introductory article explains the underlying concepts and main theoretical roots from which the ART of physical inactivity and exercise was developed (field theory, affective responses to exercise, automatic evaluation, evaluation-behavior link, dual-process theorizing). We also summarize the empirical tests that have been conducted to refine the theory in its present form.  相似文献

The exercise psychology literature includes an intriguing, albeit not frequently discussed, paradox by juxtaposing two conclusions: (a) that exercise makes most people feel better and (b) that most people are physically inactive or inadequately active. In this article, we propose that this might be an artifact rather than a paradox. Specifically, we question the generality of the conclusion that exercise makes people feel better by proposing that (a) occasional findings of negative affective changes tend to be discounted, (b) potentially relevant negative affective states are not always measured, (c) examining changes from pre- to postexercise could miss negative changes during exercise, and (d) analyzing changes only at the level of group aggregates might conceal divergent patterns at the level of individuals or subgroups. Data from a study of 12 men participating in a 90-min walk-run protocol designed to simulate the demands of sports games (e.g., soccer) are used to illustrate these points.  相似文献
